1. Advocacy and Leadership

Provide advocacy and leadership in primary care settings to meet the needs of culturally diverse populations, providers, and environments.

​Leadership and advocacy are foundational aspects of the nursing profession to provide quality care to diverse cultures and patient populations while ensuring optimal patient outcomes. Health promotion and collaboration are equally essential in aiming to meet the needs of all patients in any environment.  Examples of outcome measures:​

  • Demonstration of leadership, advocacy and research in collaborative healthcare teams, i.e. clinical practice and preceptorships​

2. Technology & Information Systems

Manage technology and information systems to enhance primary care delivery.

It is essential for the APN to be familiar with how to access, manage and utilize technology and informations systems to provide high quality and effective patient care. Throughout the course of my career, advanced education and clinical experiences, I have had the opportunity to utilize various tech and IS platforms such as Microsoft Excel, Cerner, Greenway and Athena. Below are examples of clinical data graphs I have been able to establish through the use of such platforms.​

​Examples of outcome measures:

  • In addition to prior nursing experience, current objectives met through clinical experience and rotations in NURS 7110, 7330, 7440, 7550, 7920

  • NURS 7920 Completion of Billing and Coding Module and Evaluation

  • Completion of all sections of Clinical Tracking Logs in NURS 7330, 7440, 7550, 7920

  • Appropriate and efficient use of EHRs and Information Systems to analyze clinical data

​3. Research Utilization

Utilize research to improve health care quality and clinical outcomes.

​Evidence-based research in the field of healthcare empowers the healthcare provider in providing quality care for optimal patient outcomes. Furthermore, this also empowers the patient to be able to be a more active partner in decision-making and care maintenance. Knowledge is power, and as advanced healthcare nurses we have been prepared with the necessary tools and competencies to analyze, prepare and utilize quality healthcare data to facilitate optimal patient outcomes.

  • NURS 7430 Research development using the Johns Hopkins Evidence-Based Practice Model

4. Learning, Standards & Mentorship

Foster lifelong learning, professional standards, and mentorship with patients, families, and healthcare professionals.

​In an ever-changing world of healthcare, it is necessary for all healthcare professionals to maintain competencies and professional standards to ensure quality care. Ensuring continued education is a lifelong process to remain current in practice standards while empowering patients with the knowledge needed to be an active and accountable participant in managing their own healthcare.

​Examples of outcome measures:

  • ​EBP research and support for use of Mishel’s Uncertainty in Illness Theory for Application in Advanced Clinical Practice - this particular theory provides a guideline for ensuring adaptive measures throughout the disease process through successful patient education and mentorship

​

  • ​NURS 7230 Pathophysiology Group collaboration PPT project on the study of Macrocytic Anemia as related to incidence, etiology and relevance to improve patient clinical outcomes​

  • ​Become familiar with the Alabama State Nurse Practice Act and guidelines, including scope of practice and prohibited procedures

5.​​​ Health Promotion & Disease Prevention

Deliver comprehensive primary health care emphasizing health promotion and disease prevention across the lifespan.

One main attribute of the PCNP is to provide high-quality care while emphasizing health promotion and disease prevention across the lifespan of diverse patient populations. This is an emphasis of care established by the AUCON program while intertwined into all coursework I have completed and further instilled into every patient encounter. Each patient interaction is unique and we must often meet patients where they are. Recognizing this aspect of care is often as essential as providing the correct diagnostics and treatments. Also, the utilization of SMART Goals can further impact a patient's successful outcomes and compliance. These elements are largely represented in a provider's approach and plan of care and documented in the medical record. â€‹

  • Successful completion of all clinical courses with excellent preceptor evaluations

​6. Culturally Competent Care

Implement culturally competent care focusing on quality improvement and safety while addressing socio-economic, political, ethical, and diversity factors.

​The most effective healthcare providers should become proficient and knowledgeable in understanding gaps and barriers to healthcare. The PCNP should exhibit culturally competent care while addressing barriers and understanding factors of diverse populations. It is essential to recognize and discuss such barriers and assist the patient with finding solutions to for quality care. Additionally, the competent care provider should promote and practice health equity for all populations.

Examples of outcome measures:

  • While in 7920 Primary Care Clinicals, the NP, nurse and I worked to assist underserved patients with gaining not only needed healthcare resources but also financial assistance and food security resources through such means as :

    • completion of Social Security/Disability paperwork​

    • SARHA resources/Meals on Wheels delivery

    • Home Health/Hospice

    • Parking permit paperwork

  • NURS 7250 Promotion of health and resource equity for diverse populations through advocacy measures as demonstrated in a letter to the state legislature

  • NURS 7250 Video discussion and PPT presentation regarding the topic of Medicaid Expansion & Health Delivery for Alabamians

7. Principles of Theory & Leadership

Apply theoretical foundations and leadership principles to advance primary care practice.

The nursing profession has been guided by women who were pioneers who shaped the perception of our profession today. The evolution of our profession has certainly been shaped by such leaders as Roy, Mishel, and Peplau, among many others. The study and application of nursing theory can provide a necessary foundation for such practice in both clinical and didactic applications.

Examples of outcome measures:​​

  • Study and implementation of Peplau's Theory of Interpersonal Relations in general and every day nursing practice of diverse patient populations

                                       

  • Study and implementation of Roy's Adaptation Model in general and every day nursing practice for breast cancer population

8. Ethical Decision-Making

Integrate ethical principles in decision-making for individuals, populations, and healthcare systems.

​Ethical practice and decision-making is not only foundational to healthcare, but also exhibited in the character of the healthcare provider. This is essential aspect of not only practice, but most importantly, the provider, will serve to guide the practitioner in upholding the highest standards of practice. Surrounding one's self by such like-minded team members promotes respectability and accountability while demonstrating and promoting quality care and advocacy for all patients.

Examples of outcome measures:

  • ​NURS 7250 Collaboration with fellow MSN candidate and personal injury lawyer on the Analysis of Ethical & Economic Perspectives on Health Policy

  • NURS 7250 Healthcare Policy & Advocacy - Research and proposal for legislation expansion in the increased advocacy of the state Medicaid program to ensure greater and more equitable access to healthcare for Alabamians

9. Effective Teaching & Learning​

Use effective teaching/learning principles to guide patient education and health promotion.

​The effective use of teaching and learning principles is essential to best health outcomes. Effective teaching can reduce fears and anxiety which is essential in a holistic approach to quality patient care measures and a reflection of patient compliance. Successful utilization of these elements can have a long-term effect on patient well-being and also reinforces rapport and confidence in the patient-provider relationship. As a seasoned nurse, I have had the opportunity to demonstrate such principles time and time again and been able to personally observe the direct clinic impact of these elements of quality care.​

Examples of outcome measures:

  • ​NURS 7550 Adult SOAP - Patient education for episodic encounter

​​

  • Successful completion of the Breast Health and Early Detection Health Promotion Project as evidenced by increased patient knowledge and compliance of screening mammography

  • Research, composition and completion of the Breast Health and Early Detection Health Promotion Project throughout NURS 7440, 7550, and 7920
